Clutch adjust on '85 240 and Knob replacement?

I'm having a hard time getting the old girl into reverse. It grinds. The clutch works well, but engages on the floor. How do I adjust it? Also, how do I get the old, ratty, worn out gear shift knob off and the new one on? I have the 4 spd with the button in the middle for 5th gear.
